The publisher and standard practice determine the order in which parts of the manuscript are submitted. After purchasing the “Online Premier Pro” publishing package from iUniverse, the “Manuscript Preparation Guide” was downloaded. This document provided all submission guidelines, including page and text formatting, and organization of the manuscript.

The bulk of the manuscript is organized as follows: Front Matter, Book Block and End Matter. The End Matter may be viewed by clicking on the tabs to the left. End matter consists of:

Appendices: Data that might disrupt the flow of the book but could help the reader understand the text. The appendix could include references, tables, background research, sources, and reports.

Notes: Any notes needed to expand on the text should be placed here and organized by chapter. I didn’t include a notes section. I felt “Creative Design for Health Care Reform” told the story. This web site is used to expand on the issues raised in the book and offer extensive references for the concepts presented.

Glossary: Alphabetically arranged terms and jargon with their definitions. New words or phrases, e.g., “Medical-Industrial Complex,” would be listed and defined here.

Resources: A list of web sites for products and organizations the reader might be interested in.

Bibliography or References: A list of sources for information used in the book. These should be arranged alphabetically by the authors last name. The standard layout defined by the “Chicago Manual of Style” should be used.

Index: A list of words, terms and names of people mentioned in the book. My publisher, iUniverse, offers an automated service to generate this information as part of their publishing packages.

Author Biography: Your publisher may want a short author biography and picture for the back cover. I have included a sample picture (“I told you so”) and a short author biography.
