Creative Design for Health Care Reform tells my story of discovering the causes and cures for our health care system crisis. Some of the short term cures, e.g., diabetes disease state management presented in Part Three-Chapter Six, fit into the flow of the book. Other elements critical to health care reform, did not. Susan Montgomery, my professional medical editor, suggested that I move the business plan to the Appendix. Appendix I presents the business plan for developing the Health Care System for Quality or HC-SQ. It seemed reasonable to refer to the business plan as “HC2” in some places. The first document seen below is the proposed organization chart from this appendix. It defines functions, relationships and reporting lines, both solid and dotted. This is a straw document that may require some revision before it is put into action but it does identify the key players and their responsibilities. If America is to pull out of our Second Great Depression the vision must reach fruition as soon as possible.

Creative Design for Health Care Reform is not a picture book. It contains very few charts and tables. The organization chart for the Appendix is reproduced below. If you want to know how it helps tell the story of our health care crisis and illustrate my vision of the future, you will have to read the book. Enlightenment is relatively inexpensive but does take a prepared mind and some effort.
